Eating and pregnancy go hand in hand. Even though it doesn’t seem fair to be pregnant

and not get to eat what and when you want it is healthier for you and your baby. The old

adage of eating for two is true but many women overeat when they are pregnant and gain

more weight than they should.

The more food that you eat in one sitting the harder it is going to be for your body to

produce enough insulin to turn the sugar into energy. Not only is what you eat important

so is the portion sizes. Your doctor or dietician will provide you with a meal plan and it

is wise to follow it as closely as possible.

Since you will be eating smaller meals, you are going to need to eat more frequently to

keep your energy up. The best way to do this is plan on eating six smaller meals

throughout the day. You will keep a steady stream of nourishment going into your body

and if you eat at the same time each day it will make it easier for your body to regulate

insulin production and use.

A schedule that works for many women is to eat a small breakfast and then continue to

eat approximately every two to three hours. This will include a mid-morning snack,

lunch, an afternoon snack, dinner, and a bedtime snack. If you are still finding you are

hungry in between meals or are finding ketones when you test your urine, consult with

your doctor or dietician. They are sure to have suggestions to help you make changes

that will work better for you. Such as eating more protein at meals or filling up on more

vegetables (something that can be eaten as a free food at any time).

Dinner is the meal of the day when people like the most variety. You don’t want to eat

the same thing each night (pregnant or not). Here is the time to be creative. But a

cookbook or borrow one from the library to stock up on good ideas for dinner

combinations that fit in with your diabetic diet.

The dinner meal traditionally consists of a starch (whole grains, potatoes, and rice), a

vegetable, and a protein. You can be creative in how you combine these elements but

take the appropriate portions. Your dietician will give you guidelines on is considered a

proper portion of meat and other food groups. If you are having difficulty with this, you

may want to consider purchasing or borrowing a food scale until you learn how to judge a

portion size by sight.

There are numerous dinner combinations available by mixing up protein choices (fish,

steak, pork chops, and chicken) grains (brown rice, potatoes, pasta, and couscous), and

the various ways to cook vegetables (raw in a salad, steamed, grilled, or boiled).

If you are going out to eat for dinner, don’t be afraid to ask for your food done differently

than what the menu offers. Ask for substitutions and sauces on the side where

appropriate. Most restaurants are flexible and are willing to accommodate special dietary

requirements especially for pregnant women with diabetes.

Depending on when you are diagnosed with gestational diabetes during your pregnancy

(most likely between weeks 24 and 28) you are going to have many weeks of watching

what you eat ahead of you. If you find a meal that you like and works well with your

blood sugars you may be tempted to eat it again (and again and again).

You are going to reach a point where you do not want to even think about a piece of toast

with peanut butter again. And when you do, here are some ideas for a diabetic friendly

breakfast:

Go for quality foods because as you can see, meal sizes are going to be small. By

combining your breakfast foods with a protein you will assist your body in processing the

sugar. The added benefit of the protein is be satiating for a longer period of time. If you

fill up on carbohydrates (which is very easy to do at breakfast) you are going to be

hungry sooner and have a higher blood sugar for your next reading.

As breakfast is going to set the tone for the rest of the day, don’t cheat. If you have a

high-sugar cereal for breakfast, your blood sugar will be elevated for the rest of the day.

After indulging you will have to make up for it during subsequent meals by having less to

even out your blood glucose levels.

There are two kinds of diabetes, type 1 diabetes and type 2 diabetes. The first type is also

known as juvenile diabetes and is usually diagnosed in childhood. It is the body’s cells

and the pancreas’ inability to produce enough insulin. In type 2 diabetes there is not

The type of diabetes the majority of children are diagnosed with is type 1 diabetes. Type

2 diabetes is associated with the disease when it is diagnosed in adults – also called adult-

onset. There are cases of children being diagnosed with type 2 diabetes and the numbers

are growing.

diabetes as well – increased urination (frequency), unquenchable thirst, and

feeling very lethargic

Some children will show none of the above signs and it can be hard to diagnose them

with type 2 diabetes. Annual check-ups with your doctor are important. Discuss with

your doctor your family’s medical history. If there is diabetes present in your family

members your doctor may want to run routine tests even in the absence of additional

signs or symptoms.

The treatment for this type of diabetes is sometimes the same as type 1 diabetes. In some

patients, they will have to take an insulin injection every day. But, with type 2 diabetes,

depending on the severity of the disease, it is also possible to control blood glucose levels

with diet and exercise alone.

Planning on a family vacation? Don’t worry trips and other family outings do not have to

be restricted because your child has been diagnosed with juvenile diabetes. You can do it

all still but there are some extra preparations and planning that will have to happen first.

The preparations you make is going to be dependant on what kind of trip you are

planning, for how long and the activities that you will be participating in.

It is a good idea if you are going out of town on an airplane or by car to get some

documentation from your doctor. You should get a letter that explains your child’s

condition and an extra prescription for any unforeseen eventualities. If you are going on

a lengthy plane ride, you will need the letter to get permission to bring your child’s

medicine and syringes onto the airplane with you instead of storing them in your luggage.

If you are unsure what special arrangements you are going to need to make, speak to your

doctor. If you are going on a vacation that will involve lots of extra physical activity

(such as camping or hiking), be sure that you bring extra food to replace the energy that is

going to be used up. If there is going to be less physical activity, more frequent testing of

blood sugar levels will be necessary to make sure too much insulin isn’t being used.

When traveling a distance that involves crossing different time zones, be prepared to

make additional adjustments to your child’s eating schedule. No matter what the clock

says, your child is going to need their insulin and food on their body’s time. Again, your

doctor can help you make a plan for this adjustment and it will be based on the length of

your trip, the difference in time zones, and your child.

Although there is no known cause for juvenile diabetes there are risk factors that can

contribute to the likeliness a child will be diagnosed with the disease. As some forms of

type 1 diabetes are an autoimmune disease you can be at a higher risk if you have already

been diagnosed with a different autoimmune disease. There are also some conditions

surrounding a mother’s pregnancy and labor than could contribute to the diagnosis of

juvenile diabetes.

If your child has been diagnosed with one of these autoimmune disease he or she is

considered at a higher risk for diabetes in childhood:

* If your child has had one of these viruses: hepatitis, mumps, or CMV disease

* Thyroid problems known as hypothyroidism or hyperthyroidism (or Graves

disease)

* Celiac disease

There has been some evidence that has shown that a child born to a mother over the age

of 35 could be at higher risk for developing type 1 diabetes. This is not conclusive and it

is not to say that a child born to a younger mother is not at risk as well. Some studies

indicate that a mother who had pre-eclampsia during pregnancy will give birth to a baby

with a higher risk of being diagnosed – but this is not a proven fact.

Other risk factors include race – people from Northern Europe or areas of the

Mediterranean – are considered at higher risk than other races. Environmental and

dietary factors can play a role too. If a child is under a lot of stress it is considered a

reason why he or she may go onto develop type 1 diabetes. Dietary risks factors include

high levels of dairy and nitrosamines (used as a preservative in some meats and cheeses).

Exposure to toxins is considered a risk factor too.

bring about mixed emotions. You don’t want your child to be pitied, you don’t want

When you first tell people they will either provide positive support, be neutral, or there

may be some negative reactions. The first two reactions are not a problem, you may get

careful of. You know it is a serious disease and you need to be sure your child is aware

of it too but you don’t need outsiders bringing doom and gloom into the situation. There

are tools and support to manage diabetes and your child can still expect a full and happy

life.

The easiest way to deal with negative reactions from friends and family members is to

educate them. Most times they have an incorrect impression of what diabetes is, how it is

managed, and the long-term effects. It is serious but damage can be minimized with

good control.

Your child may be nervous telling his or her friends too – afraid that they will be looked

at as different. Most children will have a neutral reaction or none at all. It is not

something that will affect friendships and it is not a problem. If your child does suffer

from teasing or taunting because they cannot have candy or some other silliness – let

them talk to you about it and express their feelings. Some of their friends may have

questions and some may not. If there is a concern with how your child is being treated or

how the diagnosis will be received speak with your child’s teacher.

In the beginning especially you may feel guilt because you are constantly saying no to

your child. It is for their own good that they can’t have treats whenever they want but it

doesn’t make it any easier. Steel yourself against the cries, begging, and whining that

may ensue and know that it will get easier as time goes on. Change is difficult for many

including kids.

As your child get older and has more experience under their belt, involve them in the

process of managing their diabetes. This will help them feel more in control of their

disease and as the same time prepare them for the time when they are going to be on their

own and have to take care of their own injections and blood glucose monitoring.

In many children with juvenile diabetes a period occurs shortly after being diagnosed

they go through what is commonly called as the honeymoon period. This is a time when

your child’s blood sugar levels will return to normal without the aid of additional insulin.

It is important to remember that this happens in a lot of children and does not mean that

the disease has gone away. The pancreas is still trying to do its job and is working

overtime to make insulin for your child’s body. Following are some guidelines to follow

when this happens.

The length of the honeymoon period is not the same for everyone. In one child it can last

months while in another child it can feasibly last for over a year. It all depends on how

much insulin your child’s pancreas can produce and how long it can keep up that rate of

production. It will be hard to figure out how much if any insulin your child needs during

this time when their blood sugar levels are normal. Constant monitoring of the blood

glucose levels is still required, because you will not know when the honeymoon period is

over otherwise.

Keep in close contact with your health care provider. He or she will help in determining

what the best plan is to follow during the honeymoon phase. Some may even want you to

give minute amounts of insulin daily in order to keep your child used to the injections and

your child’s body accustomed to the additional insulin it will need.

It can be emotionally hard for your child during the honeymoon phase. Everything will

seem back to normal and they are going to want to resume their life like it was prior to

diagnosis. It is important to remain on the diabetic diet and continue monitoring during

this time.

management over to them. It may seem a bit intimidating to you to let your child take

process from the beginning. Their input and the ability to make some decisions will help

them feel in control of a situation they would rather not be in.

The biggest point to stress and make sure your teenager understands is how important it

is to keep proper control of their diabetes. Juvenile diabetes is a serious disease and it has

serious complications if blood sugar levels are not kept under control.

Discuss with your child different scenarios that are sure to arise and try and come up with

solutions or ways to manage them. Drinking can have a negative affect on blood glucose

levels and it is important that a young adult realize the dangers. When they reach legal

age, they need to know how drinking can affect them and ways to incorporate that into

their life if they so choose.

As a parenting, letting go is a hard thing to do but it is necessary to create independent

adults. Trust your child to make the right decisions for their diabetes and be there to

guide them when necessary.

हमारे शरीर में गले में सांस नली ज्तंबीमं से तितली के पंखो के आकार की लिपटी हुई  ग्रंथि होती है जिसे थॉयराइड कहते हैं. यह थॉयराक्सिन हार्मोन बनाती है।शरीर में किसी संक्रमण (इंफेक्शन) के कारण शरीर की प्रतिरोधी क्षमता (इम्यून सिस्टम) द्वारा एंटीबॉडीज़ बनाई जाती है जो संक्रमण को समाप्त करती है। कुछ व्यक्तियों में जेनेटिक कारणों से संक्रमण के पश्चात शरीर की प्रतिरोधी क्षमता संक्रमण को तो समाप्त करती ही है, लेकिन साथ ही थॉयराइड ग्रंथी को भी क्षति पहुँचाती है। इस क्षति के परिणामस्वरूप थॉयराइड ग्रंथी सामान्य से कम हार्मोन बनाने लगती है।

यहाँ तक कि पाँच हजार में एक बच्चे में तो यह ग्रंथि जन्म से ही अनुपस्थित होती है जिसके कारण हार्मोन बिल्कुल नही बनता एवं इन बच्चों को जीवन भर थायरॉक्सीन की आवश्यकता होती है जिसे टेबलेट के जरिये दिया जाता है।

हार्मोन कम बनने का सम्बन्ध खाद्य पदार्थ से नही होता और न ही किसी विशेष खाद्य पदार्थ के सेवन करने से हार्मोन की पूर्ति की जा सकती है। हायपोथॉइराडिज्म थॉयराइड हार्मोन की कमी से होता है अत: एक व्यक्ति से दूसरे व्यक्ति में नही फैलता।

थॉयराइड हार्मोन की टेबलेट कब लेना चाहिये ?

इसकी टेबलेट सुबह खाली पेट लेना चाहिये क्योंकि खाली पेट यह शीघ्र आंतो द्वारा अवशोषित होती है। इसके अतिरिक्त एक निश्चित समय टेबलेट लेने का होना चाहिये जिससे प्रतिदिन टेबलेट लेने की नियम बन जाये। उसके लिये सुबह का समय उपयुक्त होता है।  इसलिये इसे खाली पेट लेना चाहिये।

टेबलेट लेने के कितनी देर बाद से ही कुछ ले सकते है ?

टेबलेट लेने के पाँच मिनिट बाद से ही चाय, दूध आदि का सेवन कर सकते हैं।

यदि सुबह टेबलेट लेना भूल जाये तो क्या करना चाहिये ?

यदि सुबह टेबलेट लेना भूल जायें तो दिन में कभी भी याद आने पर इसे ले सकते हैं।

क्या थॉयराइड की टेबलेट जीवन भर लेना होगी ?

जी हाँ, इसे जीवन पर्यन्त लेना आवश्यक है, क्योंकि यह दवाई न होकर हमारे शरीर में प्रतिदिन बनने वाला हार्मोन है। चूकि थॉयराइड ग्रंथि जीवन पर्यन्त थॉयराक्सिन बनाती हैं, अत: इसे जीवन भर लेना आवश्यक है।

इस टेबलेट के दुष्प्रभाव क्या हैं ?

इस टेबलेट की डोस का निर्धारण थॉयराक्सिन हार्मोन की कमी के आधार पर किया जाता है। रक्त की जाँच कर हार्मोन की कमी का पता लगा कर कमी को पुरा करने के लिये टेबलेट की उचित मात्रा दी जाती है। यदि सही मात्रा ले रहे है तो कोई दुष्प्रभाव नही हो सकता। निर्धारित मात्रा से आधिक सेवन करने सेें हार्मोन की अधिकता हो जायेगी और अधिक हार्मोन से नुकसान हो सकते है। निर्धारित मात्रा से कम लेने पर रोग के लक्षण पुन: प्रकट हो सकते है।

थॉयराइड हार्मोन के लिये रक्त की जाँच कब कराना चाहिये ?

रक्त की जाँच का उद्देश्य व्यक्ति को कितने हार्मोन की आवश्यकता है यह जानना है।  प्रारम्भ में उपचार करने पर टेबलेट की सही मात्रा निर्धारण के लिये 3–6 माह में रक्त की जाँच की जाना चाहिये। इसके पश्चात् चिकित्सक की सलाह पर समय समय पर टेबलेट की उचित मात्रा निर्धारण के लिये की जाना चाहिये।

क्या उस दिन टेबलेट लेना चाहिये। जिस दिन रक्त की जाँच कराना होती है ?

उस दिन रक्त का नमूना जाँच के लिये देने के बाद टेबलेट अवश्य लेना चाहिये। अर्थात जाँच वाले दिन भी टेबलेट लेना चाहिये

उपचार के दौरान यदि अन्य कोई रोग जैसे बुखार, दस्त लगना, सर्दी खाँसी आदि हो जाये तो क्या थॉयराइड हार्मोन की टेबलेट उस दौरान निरन्तर लेना आवश्यक है ?

चूंकि हमारे शरीर में थॉयराइड ग्रंथि सतत् थॉयराइड हार्मोन बनाती है अत: उसकी कमी की पूर्ति सतत् होना आवश्यक है। अत: अन्य रोग हो जाने पर भी उस रोग के उपचार के साथ थॉयराइड हार्मोन की टेबलेट लेना आवश्यक है। इसे लेना बंद नही करें।

क्या रक्त में थॉयराइड हार्मोन की जाँच के लिये खाली पेट होना आवश्यक है ?

कुछ वर्ष पूर्व एलिसन, रेडियो इम्यून ैण्।ण् आदि से थॉयराइड हार्मोन की जाँच की जाती थी, जिसके लिये खाली पेट होना आवश्यक था। किन्तु आज कल अत्यन्त उच्च तकनीक से जाँच के कारण खाली पेट होना आवश्यक नही है।

क्या होम्योपैथी या आयुवेर्दिक पद्वति से हायपोथॉइराडिज्म़ का उपचार हो सकता है ?

इसकी चिकित्सा में जो टेबलेट दी जाती है, वह थॉयरॉक्सीन ही है अर्थात यह दवा न होकर शरीर में बनने वाला प्राकृतिक रसायन ही है। अत: हायपोथायराइडिज़्म में थॉयरॉक्सीन टेबलेट तो इसकी एक तरह से प्राकृतिक चिकित्सा है। अत: किसी होम्योपैथी या आयुर्वेदिक या अन्य किसी भी दवा से हायपोथायराइडिज़्म की चिकित्सा नहीं की जा सकती है। और फिर जब थॉयराइड द्वारा बनाया जाने वाला हार्मोन थॉयरॉक्सीन ही उपलब्ध है तो फिर किसी और दवाई की आवश्यकता ही नहीं है। चूँकि थॉयरॉक्सीन हार्मोन टेबलेट के रूप में दिया जाता है एवं काफी सस्ता है, बड़ी आसानी इसके डोज़ को एडजस्ट किया जा सकता है, इसलिये चिकित्सा विज्ञान में इससे संबंधित किसी नई दवा पर शोध भी नहीं किया जाता।

क्या योग या व्यायाम द्वारा उपचार संभव हैं ?

हायपोथॉइराडिज्म़ में थॉयराइड ग्रंथि क्षतिग्रस्त होने थॉयराइड हार्मोन कम बनता है। हार्मोन की कमी की पूर्ति हार्मोन द्वारा ही संभव है जो योग या व्यायाम नही कर सकता। अत: योग एवं व्यायाम का उपचार में कोई योगदान नही है।

क्या हार्मोन की कमी वाले व्यक्ति गर्भधारण योजना बना सकते है ?

हार्मोन की कमी वाले तभी गर्भधारण कर सकते है जब रक्त में उनका हार्मोन स्तर सामान्य हो। चूंकि गर्भधारण के दौरान बच्चे के विकास के लिये हार्मोन की आवश्यकता होती है जो उसे माँ से मिलता है। एक बार गर्भधारण के 4 माह और 7 माह में गोली की मात्रा का निर्धारण करना चाहिये ताकि माँ का हार्मोन स्तर सामान्य बना रहे और गर्भ में विकसित हो रहे बच्चे में थॉयराइड हार्मोन की कमी न हो। यदि गर्भवती महिला में थॉयराइड हार्मोन कम होगा तो उससे गर्भ में विकसित हो रहें बच्चें का मानसिक और शारीरिक विकास प्रभावित होगा।

डायबिटीज़ के कुछ आम लक्षण होते हैं, जिनके बारे में सब लोग जानते हैं, लेकिन लगभग 50 प्रतिशत व्यक्तियों में यह रोग बिना लक्षण के भी हो सकता है ।

कई बार तो मधुमेह का पता तब चलता है, जब इसकी वजह से शरीर के किसी अंग को गंभीर क्षति हो चुकी होती है। मधुमेह का जल्द निदान (Diagnosis) महत्वपूर्ण है, क्योंकि समय पर पता चल जाने पर चिकित्सा के माध्यम से इसके दीर्घकालीन दुष्परिणामों से बचा जा सकता है।

मधुमेह के लक्षणों को मुख्यतः तीन समूहों में बाँटा जा सकता है :


1. रक्त में ग्लूकोज़ की मात्रा अधिक हो जाना :


अगर ग्लूकोज़ शरीर में एक निर्धारित मात्रा से अधिक हो जाये तो यह पेशाब में आने लगता है, जिसकी वजह से मरीज़ को अधिक पेशाब आने की समस्या होती है । इस कारण शरीर में पानी की कमी होती है, जिससे अधिक प्यास भी लगती है। शरीर में बार-बार संक्रमण इन्फेक्शन होने लगता है। कई बार चोट लगने पर घाव ठीक नहीं होते एवं इनमें संक्रमण होता है। महिला व पुरूषो में जननांगों में फंगल इन्फेक्शन की वजह से खुजली होना भी मधुमेह का एक आम लक्षण है ।

आँखों के आगे धुँधलापन या स्पष्ट न दिखाई देना भी मधुमेह का एक लक्षण हो सकता है। रक्त में ग्लूकोज़ स्तर अधिक होने से लेंस का आकार सिकुड़ जाता है, जिसके फलस्वरूप द्रष्टि दोष होता है। ग्लूकोज़ स्तर सामान्य होने पर लेंस का आकार भी सामान्य हो जाता है एवं द्रष्टि सामान्य हो जाती है ।

यदि किसी व्यक्ति को पहले से चश्मा लगा है, तो मधुमेह होने पर चश्मे का नंबर बार-बार बदलने लगता है। बेहतर होगा कि ग्लूकोज़ स्तर सामान्य होने के बाद ही चश्मे के नंबर की जाँच कराई जाए ।

2. शरीर की कोशिकाओं के भीतर ग्लूकोज़ नहीं पहुँच पाना :

जब ग्लूकोज़ शरीर की कोशिकाओं के भीतर नहीं पहुँच पाता तो थकान, कमजोरी, चिडचिड़ाहट, हाथ पैर में दर्द, झुनझुनी, अधिक भूख लगना जैसे लक्षण होते हैं । साथ ही शरीर भोजन के ग्लूकोज़ का उपयोग नहीं कर पाता, जिस वजह से वज़न घटने लगता है ।

3. मधुमेह के शरीर पर दुष्प्रभाव :

रक्त में ग्लूकोज़ का बढ़ा हुआ स्तर शरीर के विभिन्न अंगों को क्षति पहुँचाता है, जिसके परिणामस्वरूप अनेक लक्षण हो सकते हैं, जैसे – हाथ-पैरों में झुनझुनी, सुन्न होना, सुइयां चुभना, बहुत अधिक दर्द होना, नपुंसकता, धुँधला दिखाई देना, शरीर पर सूजन आ जाना आदि ।

मधुमेह के लक्षण मधुमेह के प्रकार पर भी निर्भर करते हैं। यदि मरीज को टाईप-1 मधुमेह है, तो ऎसी अवस्था में तेजी से वज़न घटना, बार-बार पेशाब आना, बच्चों में बिस्तर गीला करना, बेहोश हो जाना प्रमुख लक्षण होते हैं ।

वर्तमान युग के तनाव भरे और शारीरिक श्रम से रहित जीवन में मधुमेह किसी को भी हो सकता है। फिर भी कुछ कारक (Risk Factors) हों, तो मधुमेह होने की संभावना बढ़ जाती है।

निम्न व्यक्तियों को मधुमेह होने की संभावना अधिक होती है :-


1. यदि वंश में मधुमेह हो :
जिन व्यक्तियों के किसी रक्त संबंधी जैसे माता-पिता, भाई-बहन आदि को मधुमेह है उन्हें मधुमेह होने की संभावना अधिक होती है।

मधुमेह होने की संभावना प्रतिशत में :

  • माता-पिता दोनों को मधुमेह है – 95%

  • माता को मधुमेह है व दादा-दादी में से किसी को मधुमेह है  – 70%

  • पिता को मधुमेह है व नाना-नानी में से किसी को मधुमेह है – 70%

  • माता या पिता दोनों में से किसी एक को मधुमेह है – 40%

  • भाई या बहन को मधुमेह है – 30%

  • मामा/चाचा/बुआ/मौसी को मधुमेह है – 20%
2. मोटापा :

मोटापे की वजह से इन्सुलिन की कार्यप्रणाली घट जाती है, अतः मधुमेह की संभावना बढ़ जाती है। पेट के आसपास मोटापे (सेन्ट्रल ओबेसिटी) से मधुमेह की संभावना बढ़ जाती है।

मोटापे का अर्थ : मोटापे को दो तरह से परिभाषीत किया गया है –

A. बॉडी मास इन्डेक्स (B.M.I.) का 25 से अधिक होना ।

B. कमर-कूल्हा अनुपात (W.H.R.) का अधिक होना।

केन्द्रीय मोटापा (सेन्ट्रल ओबेसिटी) :

पेट के आसपास अधिक चर्बी होने पर मधुमेह की संभावना बढ़ जाती है। केन्द्रीय मोटापे (सेन्ट्रल ओबेसिटी) के लिए कमर एवं कूल्हों का नाप इंच टेप से लिया जाता है और दोनों के अनुपात की गणना की जाती है, जिसे कमर-कूल्हा अनुपात (Waist-Hip Ratio, WHR) कहते हैं।

उदाहरण के लिए यदि एक पुरूष के कमर का माप 90 से.मी. और कुल्हे का माप 80 से.मी. है, तो WHR 1.12 होगा।

यदि WHR पुरू्ष में 0.9 व स्त्री में 0.85 से अधिक है, अथवा यदि कमर का नाप पुरूषो में 120 से.मी. (40 इंच) व महिलाओं में 88 से.मी. (35 इंच) से अधिक है, तो उन्हें मधुमेह की ज्यादा संभावना होगी।

3. शारीरिक श्रम का अभाव
4. अधिक कोलेस्ट्रॉल
5. उच्च रक्तचाप
6. यदि पहले जेस्टेशनल डायबिटीज़ हुआ है
7. 4 कि.ग्रा. से अधिक वज़न के बच्चे को जन्म देना
8. स्त्री में पॉलिसिस्टिक ओवरी सिंड्रोम
9. लो बर्थ वेट

जन्म के समय वज़न (Birth Weight) कम अर्थात 2 कि.ग्रा. से कम रहा हो, परंतु व्यस्क होने पर वह मोटा हो जाए।

यदि किसी व्यक्तिमें निम्न 5 में से 3 या अधिक मौजूद हैं तो उन्हें मधुमेह होने की सम्भावना अधिक होती है :

  1. कमर का नाप पुरुष में 120 से.मी. या महिला में 88 से.मी. से अधिक होना

  2. ट्रायग्लिसराईड 150 मि.ग्रा. से अधिक होना

  3. एच.डी.एल. पुरुष में 50 मि.ग्रा. से कम या महिला में 40 मि.ग्रा. से कम होना

  4. रक्तचाप 130/85 से अधिक होना

  5. खाली पेट ग्लूकोज़ 110 से अधिक होना
यदि उक्त 5 में से 3 कारक मौजूद हैं तो व्यक्ति को मेटाबोलिक सिन्ड्रोम (Metabolic Syndrome) है अर्थात मधुमेह की शुरूआत हो चुकी है।

आनुवंशिकता की उपस्थिति मधुमेह की संभावना में सर्वाधिक योगदान देती है। जिन व्यक्तियों के माता-पिता दोनों को मधुमेह है, उन्हें मधुमेह की रोकथाम के लिए किशोरावस्था से प्रयत्नशील रहना चाहिए।

रक्त में ग्लूकोज़ का स्तर जब निर्धारित मात्रा से अधिक हो जाता है, तो इसे मधुमेह कहते हैं। रक्त में ग्लूकोज़ क्यों बढ़ जाता है, व इसके बढ़ जाने से शरीर में क्या समस्याऎं उत्पन्न होती हैं, यह जानना मधुमेह को समझने के लिये आवश्यक है।

रक्त में ग्लूकोज़ स्तर बढ़ने का कारण इन्सुलिन नामक हार्मोन की मात्रा में कमी या इन्सुलिन की कार्यक्षमता में कमी है। इन्सुलिन एवं ग्लूकोज़ के संबंध को समझने के लिए शरीर की कार्यप्रणाली को समझना आवश्यक है।

शरीर की कार्यप्रणाली : हमारा शरीर बहुत सी छोटी-छोटी इकाइयों अर्थात अति सूक्ष्म कोशिकाओं से बना होता है। हर कोशिका के भीतर विभिन्न रासायनिक प्रक्रियाऎं होती हैं, जिसके लिये उर्जा की जरूरत होती है, और ग्लूकोज़ उर्जा की इस जरूरत की पूर्ति करता है। एक तरह से शरीर की कोशिका के लिए ग्लूकोज़ वैसा ही है, जैसे कार के लिए पेट्रोल । शरीर की कोशिकाएँ ग्लूकोज़ के बिना काम नहीं कर सकती हैं। भोजन का उददेश्य शरीर की कोशिकाओं को ग्लूकोज़ प्रदान करना एवं उनकी अन्य आवश्यकताओं की पूर्ति करना है।

हमारे भोजन में उपस्थित कार्बोहाईड्रेट आँतों में पाचन के पश्चात ग्लूकोज़ में बदलता है। यह ग्लूकोज़ रक्त में पहुँचता है और रक्त वाहिनियों के जरिये शरीर के विभिन्न भागों में पहुँचता है, जहाँ से यह शरीर की कोशिकाओं के भीतर पहुँचता है ।

रक्त में ग्लूकोज़ भोजन के माध्यम से पहुँचता है, परंतु जब हम खाली पेट होते हैं तब लीवर रक्त में ग्लूकोज़ पहुँचाता है। अर्थात खाली पेट होने पर भी रक्त में ग्लूकोज़ लगातार पहुँचता रहता है। हमारे शरीर में लीवर ग्लूकोज़ का स्टोर हाऊस है, जिसका कार्य खाली पेट की स्थिति में ग्लूकोज़ को रक्त में लगातार भेजना है।

रक्त में ग्लूकोज़ पहुँचने के बाद इसे शरीर की कोशिकाओं में पहुँचाने की आवश्यकता होती है। कोशिकाओं में एक दरवाजा होता है, जिसे ग्लट-4 (Glut-4) कहा जाता है। सामान्य स्थिति में यह दरवाजा बंद होता है। ग्लूकोज़ कोशिकाओं के भीतर तभी प्रवेश कर सकता है, जब यह दरवाजा खुला हो ।

शरीर की कोशिकाओं में मौजूद इस दरवाजे के खुलने पर ग्लूकोज़ कोशिकाओं के भीतर प्रवेश करता है व उर्जा प्रदान करता है। रक्त से ग्लूकोज़ को कोशिकाओं में प्रवेश दिलाने की प्रक्रिया काफि जटिल है, और यह तथ्य इन्सुलिन व ग्लूकोज़ की रिश्तेदारी का आधार है। ग्लूकोज़ को कोशिकाओं के भीतर प्रवेश दिलाने का काम इन्सुलिन करता है। चित्र में दिखाये अनुसार, हमारे रक्त में पहुँच रहे ग्लूकोज़ को, इन्सुलिन, एक दरबान की तरह, कोशिकाओं के भीतर प्रवेश दिलाता है और यह ग्लूकोज़ ही हमें उर्जा प्रदान करता है।

इन्सुलिन क्या है ?

हमारे पेट में अमाशय के पीछे अग्नाशय पैंक्रियाज़ ग्रंथि होती है। इस ग्रंथि में इन्सुलिन बनाने वाले ऊतक को आइलेटस आफ लैंगरहंस (Islets of Langerhans) कहा जाता है। इन आइलेटस में बीटा सेल्स होती हैं, जो इन्सुलिन नामक हार्मोन का निर्माण करती हैं।

जब भी रक्त में ग्लूकोज़ का स्तर बढ़ता है, तो इसकी जानकारी बीटा सेल्स को मिलती है और वे इन्सुलिन का स्त्राव करती हैं। इस तरह ग्लूकोज़ स्तर एवं इन्सुलिन का स्त्राव एक दूसरे पर आधारित हैं। यह इनसुलिन रक्त वाहिनियों में प्रवाहित होकर शरीर के विभिन्न हिस्सों में पहुँचता है। शरीर की कोशिकाओं से जुड़कर यह इन्सुलिन ग्लूकोज़ को भीतर प्रवेश दिलाता है और अनेक जैविक क्रियाओं को नियंत्रित करता है।

इन्सुलिन कोशिकाओं पर मौजूद दरवाजा अथार्त ग्लट-4 को खोलता है, जिससे ग्लूकोज़ शरीर की कोशिकाओं के भीतर प्रवेश करता है व कोशिकाओं के लिए उर्जा निमार्ण के स्त्रोत का काम करता है। एक तरह से इन्सुलिन कोशिकाओं में लगे ताले को खोलता है, ताकि ग्लूकोज़ भीतर प्रवेश कर सके। भीतर पहुँचने के बाद चयापचय की प्रक्रिया शुरू होती है जिसके फलस्वरूप उर्जा उत्पन्न होती है, जो शरीर की विभिन्न कार्यप्रणालियों को चलाने के लिये आवश्यक है।

इन्सुलिन एक अत्यंत महत्वपूर्ण हार्मोन है जिसके अनेक कार्य हैं, जिनमें से एक है ग्लूकोज़ को कोशिका के भीतर पहुँचाना । इसके अतिरिक्त इन्सुलिन ब्लड प्रेशर, कोलेस्ट्रोल आदि के नियंत्रण में भी काफी महत्वपूर्ण योगदान देता है।

इन्सुलिन ग्लूकोज़ नियंत्रण के अलावा शरीर की अनेक जैविक क्रियाओं को भी प्रभावित करता है। अतः इन्सुलिन को शरीर की रासायनिक मशीनरी की मास्टर चाबी कहा जा सकता है।

इन्सुलिन एवं मधुमेह :

जब पैंक्रियाज़ से इन्सुलिन कम मात्र में निकल रहा हो या इन्सुलिन निकल रहा हो परंतु कार्य नहीं कर पा रहा हो अर्थात इन्सुलिन अवरोध (Insulin Resistance) की स्थिति हो, तो रक्त में ग्लूकोज़ का स्तर तो बढ़ने लगता है लेकिन, इन्सुलिन की कमी से, यह ग्लूकोज़ रक्त से कोशिकाओं के भीतर नहीं जा पाता बढ़ने लगता है। इसी अवस्था का नाम मधुमेह है।

अतः मधुमेह में रक्त में मौजूद ग्लूकोज़ शरीर की कोशिकाओं के भीतर प्रवेश नहीं कर पाता, जिसके परिणामस्वरूप एक ओर तो रक्त में ग्लूकोज़ का स्तर बढ़ने लगता है, वहीं दूसरी और शरीर की कोशिकाओं को उर्जा के लिये ग्लूकोज़ नहीं मिलता है। अर्थात मधुमेह ऎसी अवस्था है, जिसमें रक्त में अधिक ग्लूकोज़ होने पर भी भीतर की कोशिकाऎं ग्लूकोज़ से वंचित रहती हैं।

Both diabetics and obesity are linked to kidney failure. There are many people who are
